This thread is a record of incidents & fatalities. Below are the ones that I know of. Feel free to post your own!
1993: A Sheffield Omnibus Daimler Fleetline suffers a brake failure on Jenkin Road in Wincobank. The driver veers on to a grass verge to suspend the momentum and bring the bus to a stop. There were no injuries. 1997: G622NWA, a Volvo B10M Alexander PS reverses over a fitter at Greenland Road Depot, Top Park. The fitter was underneath the bus, working on the wiring. A shunter-cleaner reverses the bus back in to the parking bay, unaware that the fitter was underneath. The fitter died as a result of his injuries. A huge health and safety investigation followed and as a result, a major overhaul of depot operations was asopted. This included different stacking methods and Not To Be Moved covers placed on steering wheels of buses that were being worked on. 1997:At Midland Road Depot, Rotherham, a Reeve Burgess Renault Dodge was dropped from a suspension rig and killed a member of depot staff underneath it. The depot staff member's brother also worked at the depot and had to be held back from running to the rig and witnessing the distressing scene. January 2000: An H registered Volvo B10M Alexander PS becomes stranded in snow at Fox House. The driver revs the engine and spins the wheels repeatedly, in a desperate attempt to free the bus. Consequently, the ground below the bus ignites and the bus is set fire to and irreperably damaged. The bus driver was unharmed and returned to active duty shortly afterwards. November 2001: A young boy is killed at the Herdings terminus. He tried to run along side the bus as it left the terminus and open the emergency exit. He fell under the bus and was crushed by the wheels. 3rd March 2002: A man and woman are killed by a Plaxton Beaver 1 mobility bus on Station Road, Mosborough. It is believed they were laid in the road love making. It was late at night and the bus was returning to the depot. The woman suffered severe head trauma and both parties died at the scene. The driver was unharmed.
2002:A bus and its onboard crew are attacked by yobs throwing bricks on Handsworth Road, Sheffield. A passenger intervenes and is himself attacked. His eye had to be removed as a result of his injuries. 2008: An elderly lady is hit by an East Lancs Spryte Yorkshire Terrier at the bottom of Angel Street. It is unknown whether she survived or not but there was a considerable amount of blood on the road. December 2015: A juvenile couple are hit by 63129 on Haymarket, Sheffield. The girl was crushed by the bus's wheels and dies at the scene. The boy suffered crushed organs but survived. A prosecution employee suggested that the driver was distracted from driving at the time and had been putting his coat on during the buses journey out of service.

A few errors: 1993 - the bus didn't suffer brake failure on Jenkin Rd. It was Newman Rd, and the grassed area just before the left turn onto Merton Lane is where the vehicle was "ditched". Jan 2000 - the chance of the actual ground spontanously combusting and then setting fire to the bus (espcially in winter with snow on the ground, though in any season) is pretty much 0. Any damage caused to the road surface would have been caused by the heat given off by the bus fire.

March 2009 was the dreadful Hooton Roberts RTC which involved TM Travel YL02FKW, First SY 37260-YN07MLF and a skip lorry, in which the First bus driver sadly lost his life (37230/1430 is/was named in his memory).

I recall an H registered B10M Alexander PS breaking through the railings at Rotherham interchange in 2000. The bus was left hanging over the river. I am not sure of full details but i believe the driver had a heart attack and his foot got stuck on the accelerator.

Yes I remember the accident well, dark day for the first rotherham team especially, apparently so I am told, they found the skip driver at fault I didn't follow the investigation much, but I was sure initially they were pointing the blame at the TM driver, for pulling out of Ravenfield Lane, causing the skip lorry to hit the Solo and vere into the path of the oncoming X78. I'm sure I read they put up a defence that the vision was obscurred for the driver due to a hedge line or something along those lines, though I can't remember any outcomes from the investigation/trials. Following the accident, the TM Solo had its identity changed to FB02LBP, and the B9TL eventually reemerged with Unobus as C14UNO, with the updated Gemini 2 front.

December 2000: flic.kr/p/21DgdBI thought it was later than that, around 2004/2005 time but obviously not.

Infact, I believe its a typo on the date in the description (presumably the "taken on" date which shows 9/12/2004 is correct). The 5-digit First numbering system didn't come in while 2003 time.

Following the accident, the TM Solo had its identity changed to FB02LBP, and the B9TL eventually reemerged with Unobus as C14UNO, with the updated Gemini 2 front. At the time of the incident there was a LOT of misinformation being put about by certain parties who were trying to deflect any possible blame being attached to them particularly with reference to hiding things from the traffic commissioners. A few details. The TM driver and the skip wagon driver were both arrested at the scene and had the First driver not been killed he would also have been arrested. That is standard procedure and is in no way an indication of wrongdoing.Charges against the TM driver were dropped. I believe the skip wagon driver was found guilty of numerous offences, speeding was dropped as allegedly the speed of the skip wagon at the time of the accident could not be proven as there was allegedly no disc in the tachograph at the time. A thorough investigation of the TM bus revealed 2 minor faults, neither of which could have contributed to the accident. The farm vehicle that had spread mud all over the road causing the TM bus to skid 3 inches over the give way line was never traced. The point of impact on the TM vehicle was actually on the side road at the time of impact. No action was ever taken against TM travel despite a FULL fleet examination being undertaken by the TC in the following days apart from a requirement to have all vehicles inspected by an outside contractor within the next 45 days, but, this was connected with an application for an operators licence extension. An extension to TM travels operators licence was granted shortly after the fleet examination. The shrubbery was cut back the next day by the owner of the property it stood on. It is my understanding that this was raised by a police officer at the scene at the time as a potential issue and expanded on by others. It is my understanding that the driver of the skip wagons mobile phone records may have played a part in the prosecutions case.

That is correct Duncan, I also am led to believe that the driver of the tm travel vehicle is now working for another company within the bus industry, the skip.lorry driver got some lengthy time in jail due to the horrific accident, but Duncan is correct any major accident every party gets arrested as a matter of procedure under investigation, tm travel.got cleared of the accident but at the time a lot of speculation and blame was aimed at tm
